Center-Based Model
Pharmacy responsibilities reside within the PACE center and the PO manages its contracted pharmacy.
Pharmacy-Based Model
Medication management is transferred from the PACE center to an offsite, dedicated Grane Rx pharmacy specializing in PACE pharmacy services.
